Professional Fees for Out of Network Patients

Please take a moment to review the professional fees for out of network patients. Each patient is required to complete an initial assessment, even if transferring from another psychiatrist.

Currently, Dr. Chande is in network for Anthem BCBS, Aetna, and Wellstar Health Plan. For all other insurance plans, she is out of network and does not bill insurance directly. Patients will be responsible for full payment of professional fees at the time of service.

A “superbill” can be provided for out of network patients upon request. This bill can be submitted by you to your insurance plan for possible reimbursement. Please check with your insurance carrier regarding your mental health benefits and reimbursements prior to your appointment.
